An audition form collects what the director and stage manager need to know about each auditioner before casting: how to reach them, which roles they want and, most importantly, when they are not available.
This form includes contact details, a few yes/no questions about casting flexibility, space for experience and a weekly availability grid where auditioners shade the times they cannot attend. A separate box captures specific conflicts like other shows or exams.
How to use this template
- 1Print enough copies for every auditioner, plus spares.
- 2Add the production name and number the forms as people sign in.
- 3Ask auditioners to shade the availability grid and list every known conflict.
- 4After casting, move conflicts straight onto your rehearsal calendar.
