When someone is hurt, or nearly hurt, the stage manager needs to record what happened while the details are fresh. A clear written report protects the person involved, the production and the venue, and it shows what needs fixing so it does not happen again.
This form records when and where it happened, who was involved, the type of incident, a factual account, injury details, first aid and medical response, witnesses, follow-up actions and who was notified. Keep completed reports confidential and follow your company's or venue's own reporting rules.
How to use this template
- 1Make sure the person is safe and cared for first; fill in the form afterwards.
- 2Write only what you saw or were told, in order, without guessing at causes.
- 3Record first aid, medical response and witnesses with contact details.
- 4Notify the people listed, file the form confidentially and track the follow-up actions.
